Your tasks:
- Case management and care of complex health and nursing situations within the expert team, focusing on palliative care, pain and wound management, behavioral interventions, e.g., in BPSD, counseling, admission process, clinical assessment, emergency and crisis management
- Competent collaboration with nursing teams and day supervisors
- Professional responsibility for the nursing process
- Co-responsibility and assurance of care quality and its review in cooperation with internal and external partners
- Appreciative, professional support of residents and shaping of family work and involvement
- Contact person for family doctors, referring parties, external service providers
- Assumption of house responsibility
- Co-responsibility for the RAI process as well as promotion and support of employees
Your profile:
- Degree as a nursing professional FH or HF
- High sense of responsibility and strong service orientation
- Ability to implement new nursing knowledge around care
- Enjoyment of learning together
- Professional experience in long-term care (advantageous)
- Training as RAI-NH MDS coordinator or supervisor (advantageous)
- Flexibility and team spirit
- Independent, reliable, and structured working style
