Social Worker 50%

We are looking for a committed social worker for our location in Basel. The workplace is primarily in Basel with assignments about once a month in Bad Zurzach.

Your tasks

• Counseling: You accompany and advise patients on questions in the areas of housing, work/daily structure, social issues, finances, especially social insurance and administration as well as child and adult protection
• Support: Assist in finding suitable follow-up solutions and in discharge planning in close cooperation with patients and their relatives
• Coordination: In close cooperation with institutions, authorities, and mandate holders, you coordinate the necessary support services
• Collaboration: Be the representative of social service expertise in the interdisciplinary treatment team, consisting of medical, therapeutic, psychological, and nursing professionals. You also participate in reports and case discussions

What you bring

• Education: Completed training in social work or a basic medical education with further training in the social sector (e.g., CAS in Case Management, social insurance law, etc.)
• Experience: Professional experience in a similar or comparable environment, preferably experience in clinical social work
• Knowledge: You are familiar with social insurance (AHV/IV, etc.) and ideally also with residential care
• Personality: You are an empathetic and motivated person with high social competence and very good communication skills. As a team player, you are solution-oriented, take responsibility, work independently, and set clear priorities
• Skills: You focus on the dignity, self-determination, and social participation of patients with rehabilitative, especially neurological diseases, and professionally master the associated challenges
