Spitex Fricktal AG is a modern and innovative non-profit Spitex. On behalf of 17 municipalities, we offer our services in a catchment area of approximately 50,000 inhabitants of the upper and lower Fricktal. Our around 100 employees and 20 apprentices and students care for over 600 clients in around 140,000 assignments per year. In addition to general somatic care, Spitex Fricktal AG also offers specialized care in the areas of psychiatry and specialized palliative care.

Requirements
- Diploma in nursing HF/FH or equivalent qualification
- At least 3 years of professional experience in healthcare, ideally in the outpatient sector (Spitex, rehab, hospital, long-term care)
- Experience in intake, triage, counseling, coordination, and interprofessional collaboration
- Strong communication and counseling skills
- High social competence and empathy
- Ability to structure and convey information clearly
- Independent, responsible, and networked working style
- Enjoyment of interface work with different stakeholders (clients, relatives, service providers, authorities)
- Good IT skills and confidence in documentation
Advantageous/desired
- Further training in case management
- Additional training in social insurance law, health and process management is an advantage
Tasks
- You receive registrations, clarify the initial situation (medical, nursing, social) and record relevant data
- You assess urgency and care needs, conduct initial visits, and hand over structured to the case-responsible nurse
- You coordinate collaboration with doctors, hospitals, insurance companies, social services, and other partners
- You ensure that all documents and information are complete and support the case-responsible nurses administratively
- You contribute to the further development of intake/triage processes, internal standards, and projects and contribute to quality, customer orientation, and cost-effectiveness
