We at Spitex Uster, a business unit of the city of Uster, are looking for committed colleagues who want to ensure and advance outpatient care, support, and assistance for around 40,500 residents in the city of Uster and the municipality of Mönchaltorf together with us.

Our range of services includes both somatic and psychiatric care, counselling, domestic help and support, as well as the Spitex shop. In addition, we are a training and internship location and a member of the Gesundheitsmeile Uster training cooperation. Our mission is to improve the quality of life of our clients through professional, caring, and individual care, support, and assistance in the household. With our help, clients can remain in their familiar environment or return home earlier after a hospital stay.

To strengthen our competent Spitex team in Uster, we are looking for you immediately or by arrangement as

Spitex - Dipl. Nursing Specialist HF and Case Management Nursing 80 - 100%

Your main tasks

• You take over case management for your clients and are responsible for planning, controlling, and evaluating the entire nursing process
• You independently conduct needs assessments with interRAI HC and create individual care plans
• You are also actively involved in direct care and support clients in their familiar homes, even in complex care situations
• You take on cross-team assignments, including professional responsibility for your colleagues
• You promote the independence and quality of life of clients through targeted nursing measures
• You work interdisciplinarily both internally and externally (with specialist agencies, doctors, hospitals, and other services)

What you bring with you

• Diploma as a nursing specialist HF or FH
• InterRAI HC course or willingness to complete it
• High level of independence, good organisational skills, and resilience
• Communication skills and enjoyment of active and supportive collaboration
• Optionally own car (with mileage reimbursement by Spitex) or driving licence category B
