Your Tasks
- Care and coordination of interdisciplinary care for patients (m/f/d) with diabetes mellitus as the main or secondary diagnosis
- Focus on metabolic control as well as treatment and prevention of diabetic foot syndrome
- Guidance, advice, and support of outpatient and inpatient patients (m/f/d) with diabetes mellitus
- Conducting structured individual and group training sessions on diabetes-associated training topics
- Participation in in-house training and continuing education for internal employees (m/f/d)
Your Profile
- Completed training as a Diabetes Consultant DDG (m/f/d)
- Solid expertise in diabetology as well as willingness for continuous professional development
- Ideally experience in caring for insulin pump patients (m/f/d) and/or in wound care, especially for diabetic foot syndrome
- Independent, structured, and cooperative working style as well as pronounced teamwork skills
- High social, communicative, and pedagogical competence in patient counseling and training
- Flexibility, openness to change processes, and active willingness to take on new challenges
