Focus: Care of psychiatric or somatically ill people, with responsibility for client admissions throughout the entire care area of Spitex Region Schaffhausen.

Focus: Care of psychiatric or somatically ill people, with responsibility for client admissions throughout the entire care area of Spitex Region Schaffhausen.
In this exciting job

- you take over the organization and coordination of psychiatric and somatic client admissions, including needs assessment with InterRAI, and ensure the care and support of clients at home within the first 3 days.
- you create care plans and actively shape the care process.
- you prepare and coordinate the handover to the local Spitex team.
- you take on on-call duties during the day.
- you commit to effective interdisciplinary collaboration.
- you introduce your colleagues in complex situations.
You are successful here
- thanks to your solid training with an HF nursing diploma or an equivalent qualification.
- thanks to at least two years of practical professional experience in the field of psychiatry or somatics.
- due to your current and solid expertise.
- because you bring joy and interest in complex, dynamic situations, especially in admission management as well as organizational, coordinative, and administrative tasks.
- because you have high professional and social competence in caring for people.
- due to good IT skills, InterRAI knowledge, or Spitex experience, which is an advantage.
- because you work flexibly and independently.
- with a valid category B driving license.
