The Dielsdorf Health Center is a modern geriatric competence center with around 500 employees from 35 nations. Our services range from counseling for elderly people and their relatives to general and specialized long-term care as well as home care and support. As the largest public Spitex in the Dielsdorf district, we provide outpatient services for our nine contract municipalities. With our comprehensive care offerings, we enable people to stay in their familiar home environment as long as possible despite health limitations. In the future, it is planned to establish a night Spitex.

• Case management, assessments, and structured needs clarifications
• Ensuring the care process
• Consultation and support of clients and their relatives
• Taking over client assignments within the framework of reassessment
• Collaboration with relatives, doctors, hospitals, nursing homes, and other specialist agencies
• On-call duty every 4 weeks on weekends (clarifications and assessments from home office)
• Working hours: generally during office hours

• Completed training as a Dipl. Nursing Specialist DN II/HF/FH
• Inter-RAI HC basic course or willingness to complete it
• Several years of professional experience, ideally in outpatient care (case management or coordination functions)
• High social, methodological, and professional competence
• Very good German skills in spoken and written form (min. level B2)
• Up-to-date IT skills (MS Office, Perigon)
• Independent and responsible working style
• Driver's license category B
