Workload: 50 - 80 %

Your tasks

• Conducting initial assessments and reassessments with RAI HC as well as situational additional assessments
• Assessment of complex care situations (including palliative care) in collaboration with the nursing expert
• Ensuring the nursing process together with the nursing expert and nursing staff
• Professional support of employees in complex situations and challenges as well as in the implementation of nursing standards and instructions in addition to the nursing expert
• Maintains communication with relatives, doctors, hospitals, nursing homes, and social services
• Regular deployment as a nursing specialist HF at the clients on site
• Close and proactive collaboration in the interdisciplinary team

You bring with you

• Diplomaed nursing specialist HF with several years of professional experience in the outpatient sector
• Strong professional and social competence
• Team orientation, resilience, and flexibility
• Appreciative, clear communication and polished manners
• High sense of responsibility and quality awareness
• Good knowledge in handling digital work tools and documentation systems
• Knowledge of the interRAI HC assessment system or willingness to acquire this knowledge
• Willingness to perform on-call duty
• Driver's license category B
