Nurse Specialist for Hospital at Home

Start on March 1, 2026 or by arrangement as

• You plan your assignments together with the department management, carry out nursing care in the home environment and document it carefully
• You use your expertise for clinical assessment, monitor patients and ensure their well-being
• You work closely with doctors, hospitals, home care organizations and other partners
• You implement medical orders, support diagnostics and therapy and actively contribute your know-how
• You promote the independence of patients and specifically involve their relatives in the care process

• You have a diploma as a nurse specialist HF/FH or an equivalent recognized qualification
• You have professional experience in acute care, ideally in the field of internal medicine
• You enjoy working independently in the home environment, take responsibility and convince with team spirit as well as high clinical competence
• You show a high willingness to embrace new things, further develop structures and openly and constructively support changes
• You are interested in digital solutions and innovative care models
• You have a category B driving license and very good German skills; other languages are an advantage
