Your main tasks:
- Individual care and support of our clients in their homes
- Managing the care process and taking over case management
- Collaboration with relatives, family doctors, and other service providers
- You apply your expertise purposefully to ensure the best possible care
Your profile:
- Degree as a certified nurse HF/FH
- Flexibility and an independent, committed, and responsible way of working are among your strengths
- You are a humorous, communicative personality
- Willingness to work in different services
- Driver's license category B
