Dipl. Nurse HF/FH with Case Management (80–100%)
As a qualified nurse in case management, you take on a central role with us. You manage care processes, coordinate demanding situations, and ensure high-quality care for our clients together with the team.
You work independently, with a lot of creative freedom, and in close collaboration with internal and external specialists.
Your main tasks include:
- Taking over case management including needs assessment using interRAI HC as well as planning, implementation, and evaluation of care processes
- Ensuring high-quality, client-oriented care in the home environment
- Interprofessional collaboration with general practitioners, relatives, institutions, and other specialists
- Professional support and guidance of your team colleagues in complex care situations
- Careful documentation and active participation in quality assurance
You bring with you:
- Completed training as a certified nurse HF/FH
- At least one year of professional experience, ideally in the Spitex environment
- Enjoy working independently and responsibly
- Good IT user skills (digital care documentation)
- Driver's license category B or willingness to visit our clients in all weather conditions by e-bike
