About the job
Your Tasks
You take on more demanding inquiries from people seeking advice in 2nd level support and apply your expertise specifically by phone, in writing, and in person. You handle professional triage and efficient processing of new entries, conduct initial assessments, record concerns precisely, and initiate the appropriate next steps. You place great importance on careful documentation and ensure that the flow of information always works smoothly.
An essential part of your task is coordinating between internal and external offices, ensuring that relevant services are mediated and cases are passed on seamlessly. In addition, you actively design and coordinate a professional, transparent, and efficient onboarding process to enable new customers an optimal start.
You ensure that free UP places are filled promptly and according to demand and work closely with customer and family care as well as nursing management. You also actively contribute to the further development of counseling services and internal processes to continuously improve quality and efficiency.
Your Profile
- Education at tertiary level (FH, HF, DNII, AKP, PsyKP, KWS)
- Completed training in social work
- Expert knowledge in interRAI LTCF
- Sound knowledge and experience in health and social services in long-term and/or acute care as well as in transition processes
- Strong counseling, communication, and coordination skills
- Decision-making competence to assess care needs and care levels
