Your Tasks

• Analysis and documentation of the home, social, and professional situation of the rehabilitant as well as responsible preparation of parts of the final report 
• Advising patients on essential social law matters and supporting them with applications
• Support in coping with illness and organizing realistic assistance offers
• Collaboration with all employees of the interdisciplinary team 
• Independent presentations to patients and rehabilitants
• Group leadership in social skills training
• Participation in the establishment of the MBOR structure in the facility
• Accompanying and supporting our rehabilitants in regaining the ability to act in their own home environment
• Contact and maintenance with employers, social welfare offices, care facilities and services, cost bearers, and follow-up therapists

Your Profile

• Completed degree in social work with state recognition
• You have methodological approaches to activating and accompanying assistance for people with mainly orthopedic and psychosomatic illnesses
• Computer skills 
• Independent and reliable working style
• Empathy, organizational skills, and strong communication
• Ability for professional and multiprofessional exchange
• Resilience and ability to work in a team
• Co-responsibility for continuous improvement of therapy processes and customer satisfaction
• Experience in clinical work, ideally also in rehabilitation, is desirable but not a requirement for employment
