YOUR TASKS

• Individual and needs-based support and counseling of patients and relatives
• Organization of transfers to inpatient rehabilitation, spa stays, retirement or nursing homes
• Monitoring patients and initiating specific interdisciplinary measures to optimize patient care
• Ensuring timely discharge from inpatient acute care
• Organization of outpatient support measures and aids upon discharge
• Interprofessional collaboration with various internal and external partners and institutions

YOUR PROFILE

• Completed medical or nursing training and several years of professional experience in healthcare or as a social worker
• Knowledge in the field of social insurance is an advantage
• CAS Case Management is an advantage
• Very well-structured and solution-oriented working style as well as pronounced communication skills
• Empathy and high social competence
