Job Context
The Onex Health Center is strengthening its interprofessional team and opening a position for an IPS in primary care. The mission is part of an advanced, autonomous practice focused on the needs of the population, with a strong emphasis on collaboration and prevention.
Your main activities
You ensure advanced clinical practice including a complete health assessment, prescription and interpretation of tests, medication adjustment according to the agreement, initial management of emergencies, and referral to other health professionals.
You support patients in their care journey, inform their relatives, and ensure the quality and safety of care.
You play a key role in clinical coordination with the team, implementing specific care plans, participating in clinical meetings, and integrating evidence-based data into practice.
You are also involved in prevention, screening, health promotion, and participation in programs adapted to the community.
Finally, you contribute to nursing science research and the dissemination of results within the team.
Your profile
- You have a Master’s degree in Specialized Nursing Practice as well as a Bachelor’s degree in nursing or an equivalent qualification
- You have at least 5 years of experience in home care
- You demonstrate strong autonomy and solid clinical rigor
- You have excellent communication and interprofessional collaboration skills
- You have complementary clinical experience, which is an asset
- You have skills in research or foreign languages, appreciated
