About the job
Your tasks
- Support and accompany clients according to the care plan, promote and maintain their health and resources
- Conduct initial assessments / reassessments with RAI-HC
- Develop care planning and measures and carry out the care
- Interdisciplinary collaboration with doctors, relatives, and other institutions as well as with internally involved staff
- Perform treatment care (wound care, medication, parenteral nutrition, etc.)
You bring with you
- Diploma as a nursing specialist HF
- Broad expertise
- Willingness to take responsibility, coordination skills, and independence
- IT skills and good verbal and written communication skills
- RAI-HC course / knowledge or willingness to complete the course
- Teamwork skills, high self- and social competence
- Driver's license category B (own car advantageous)
