What awaits you
In the role as a certified nursing specialist in the assessment team, you are responsible for advising clients and their relatives from registration to handover to the care team, conducting the initial assessment, and providing initial care services until the situation is organized enough to be smoothly handed over to the responsible care team. You work throughout the Sense district and maintain close communication with all referring parties (medical profession, patient counseling, etc.) and other service providers in the social-medical field.
What you bring
You have a degree as a certified nursing specialist HF/FH and experience in outpatient care. Advanced knowledge of RAI HC is an advantage. You are professionally competent, resilient, and flexible and enjoy working independently. Your open and communicative personality significantly contributes to good teamwork as well as interdisciplinary exchange with our partners in healthcare. Working with PC and tablet is part of your daily work. You have your own car available for assignments with our clients.
