Case Management

As a case manager, you take over the leadership of the care process in the area entrusted to you. You organize and coordinate services from the areas of prevention, health care, or social assistance and align care goals with the direction of the support network as well as the responsible treating physician. You advise your clients on dealing with disease symptoms and professionally guide the nursing staff involved in care within your team.

Nursing diagnosis and planning

As part of the recurring needs assessment, you identify current or potential problem areas of your clients using RAI-HC or specific focus assessments. You then determine the relevant nursing diagnoses based on the POP classification and formulate adapted and realistic care goals together with the affected individuals. In doing so, you also plan appropriate psychiatric nursing interventions that serve to maintain or build up the resources of your clients.

Treatment care

Through your nursing activities, you support the implementation of medical therapies in everyday life, such as practicing coping strategies or guiding the handling of aggression, anxiety, or delusions. You accompany your clients during exposure training or support them in acute crisis situations. Thanks to your presence, you contribute to the prevention of acute self-harm or harm to others.

You have successfully completed training as a certified nurse HF / FH with a specialization in psychiatric nursing and are far from tired of learning new things. Your heart beats for holistic care and the well-being of people in need of care. You like to apply what you have learned in everyday work and are willing to critically question your work quality. When information is missing, you actively contribute to clarification. You master the German language in spoken and written form and are interested in further developing yourself professionally and personally.

Balanced and structured

You manage to act calmly and thoughtfully even in hectic situations. Patience and acceptance characterize your dealings with others and yourself. You are good at organizing and like to plan your workday independently. You orient yourself to the respective care plans and implement the planned measures appropriately.
