YOUR TASKS
- Individual and needs-based support and advice for patients and relatives
- Organization of transfers to inpatient rehabilitation, spa stays, retirement or nursing homes
- Monitoring patients and initiating specific interdisciplinary measures to optimize patient care
- Ensuring timely discharge from inpatient acute care
- Organization of outpatient support measures and aids upon discharge
- Interprofessional collaboration with various internal and external partners and institutions
YOUR PROFILE
- Completed medical or nursing training and several years of professional experience in healthcare or as a social worker
- Knowledge in the field of social insurance is an advantage
- CAS Case Management is an advantage
- Very well-structured and solution-oriented working style as well as pronounced communication skills
- Empathy and high social competence
