To strengthen our dedicated team, we are looking for motivated and experienced psychiatric nursing specialists HF for assignments in the Gäu region and/or in the Thierstein/Dorneckberg district, 40% - 80%
Your tasks:
- Individual and targeted outpatient care as well as case management, including careful creation and continuous maintenance of care documentation
- Advice and support for clients and their relatives in everyday tasks and challenging crisis situations
- Interdisciplinary collaboration with various professionals and institutions to ensure the best possible care
Your profile:
- Completed HF psychiatric nursing training or an equivalent qualification with experience in outpatient Spitex care
- High social competence and a team-oriented working style
- Strong counseling skills and empathy
- Confident handling of PC, tablet, and smartphone
- Driver's license and a own vehicle
