Job Description

• Listen to, guide, and inform patients of acute, outpatient, and long-term care in order to implement the appropriate and necessary steps, whether to prepare for the continuation of hospitalization (return home, placement, rehabilitation) or the follow-up of residents in our nursing home.
• You liaise with patients, care teams, and external administrations.
• You carry out procedures during individual or network interviews, by phone and by mail.

Qualifications

• Bachelor's degree in social work HES or equivalent qualification
• Professional experience desirable
• Ability to work independently and in a team
• Interest in interdisciplinary work
• Sense of responsibility and good priority management
• Flexibility
• Initiative
• Ability to integrate and adapt
