Your tasks
- Analysis and documentation of the home, social, and professional situation of the rehabilitand as well as responsible preparation of parts of the final report
- Advising patients on essential social law matters and supporting them with applications
- Support in coping with illness and organizing realistic assistance offers
- Cooperation with all employees of the interdisciplinary team
- Independent presentations to patients and rehabilitands
- Group leadership in social skills training
- Participation in the development of the MBOR structure in the facility
- Accompanying and supporting our rehabilitands in regaining the ability to act in their own home environment
- Contacting and maintaining relationships with employers, social welfare offices, care facilities and services, cost bearers, and follow-up therapists
Your profile
- Completed degree in social work with state recognition
- You have methodological approaches to activating and accompanying assistance for people with mainly orthopedic and psychosomatic illnesses
- Computer skills
- Independent and reliable working style
- Empathy, organizational skills, and communication strength
- Ability for professional and multiprofessional exchange
- Resilience and ability to work in a team
- Shared responsibility for continuous improvement of therapy processes and customer satisfaction
- Experience in clinical work, ideally also in rehabilitation, is desirable but not a prerequisite for employment
