Your tasks
- Responsibility as well as participative and motivating leadership of the employees in your team
- Active collaboration in the management team and in the quality of services.
- Regular deployment as an HF nursing specialist with customers on site
- Complex case management and comprehensive clarifications
- Making decisions and finding flexible solutions in cooperation with the
interdisciplinary team - Contact person for demanding health situations of customers
- Review and further development of nursing quality
- Conducting assessments
- Contact person for relatives and medical profession
You bring with you
- HF/DNII nursing specialist
- Professional experience, preferably in the Spitex area
- Strong professional and social competence
- Willingness to perform on-call duty
- Good communication skills and high sense of responsibility and quality awareness
- Ideally knowledge of the interRAI HC assessment system or willingness to acquire this knowledge
- Good IT skills
- Driver's license category B
