• Efficient and sustainable discharge planning to increase the satisfaction of patients and relatives as well as to reduce readmissions and rehospitalizations
• Conducting interdisciplinary patient and relative discussions
• Clarifying and organizing transfers to rehabilitation, cures, nursing homes, and care in the familiar environment
• Advising inpatient and outpatient patients and their relatives on psychosocial, financial, and social insurance matters
• Supporting patients in reintegration into family, work, and environment

• Bachelor's degree in social work or further training in case/patient management
• Ideally several years of professional experience in an acute hospital or healthcare
• Knowledge in the field of social insurance
• Good communication and moderation skills
• Team player and enjoyment of interdisciplinary work
