Your tasks

Professional care & case management

• Taking over case management in the areas of somatics and care for relatives
• Conducting needs assessments and creating individual care and support plans
• Responsibility for care process management, documentation, and service recording
• Direct care in complex and unstable care situations
• Consultation, guidance, and support of clients and relatives

Quality & professionalism

• Compliance with professional standards, guidelines, and quality requirements
• Professional support of team members in demanding care situations
• Participation in quality projects, process optimisations, and internal training
• Promotion of a professional, evidence-based nursing practice

Your profile

• Diploma as Nursing Specialist HF/FH
• Experience in Spitex and/or somatic care
• Responsible, independent, and structured working style
• High social, communication, and organisational skills
• Motivation for professional and personal development
• Driving licence category B
