Role description
- Conduct social situation assessments, follow-up and social support for patients at our centre, taking into account the psychiatric dimension
- Work within a multidisciplinary team composed of psychiatrists, psychologists, nurses, neuropsychologists tasked with ensuring integrated psychiatric and psychotherapeutic follow-ups
- Coordinate the patient’s social care in collaboration with various centre stakeholders and the network (CMS, CSR, SCTP, DGEJ, etc.)
- Conduct educational and support interventions in concrete matters (economic, administrative, professional, relational, legal, assistance in finding housing, finding a place in an institution)
- Assume the role of administrative referent, pending the establishment of an adult protection measure by the Justice of the Peace, and collaborate with the patient to avoid loss of social gains
Your profile
- Bachelor’s degree from a University of Applied Sciences in Social Work, social service orientation or equivalent qualification
- You have professional experience enabling you to work completely autonomously. Mental health experience is required
- Competence in assessing the social situation of patients presenting with mental disorders
