Case-leading Nursing Specialist in Home Treatment 60-80% (m/f/d)
Your tasks
- Case-leading care and counselling of patients in acute psychiatric "Home Treatment".
- Your responsibilities as case leader include regular home visits, treatment planning and regular treatment review in the core team, planning of system discussions, anamnesis collection, joint preparation of discharge reports
- Carrying out nursing tasks (blood sampling, ECG, medication management, vital sign monitoring, injections) and patient-related administrative work in shift work (early and late shifts)
- Taking on on-call duties
- Active collaboration within the team, with external institutions and the patients' environment
Your profile
- Training as a certified nursing specialist HF
- At least two years of experience in psychiatric nursing
- Driving licence (category B)
- Confident language and PC skills
- Open, flexible and independent personality with high social competence
