Your Tasks

You will perform assignments in basic and treatment care for our clients
You conduct needs assessments with interRAI-HC and InterRAI CMH at the client’s location
You create care documentation and individual care planning
You advise and support our clients and their relatives
You are flexible and used to working independently
You have a high level of professional and social competence

What You Bring

Training as a Dipl. Nurse with a specialization in psychiatry
Professional experience in the home care sector and with interRAI-HC and interRAI CMH
Perigon experience is an advantage
Valid driving license category B
You are resilient, flexible, and used to working independently
It is an advantage if you own a car and live in the region
