Our client is an established non-profit organisation in the field of nursing, counselling and care services. It supports people of various age groups to lead a self-determined life in their familiar environment despite health limitations. With around 160 employees at several locations, the organisation is one of the larger providers in the field of outpatient care and impresses with a dynamic working environment as well as a motivated and collegial team. To strengthen the team, we are looking for a committed and professionally competent person as a Nursing Specialist HF/FH from now or by arrangement.

Nursing Specialist HF/FH

- Conducting needs assessments according to interRAI-HC

- Independent nursing and care of clients

- Taking over case management

- Assessing care needs and care effort

- Creating and evaluating nursing documentation

- Performing and documenting interventions according to care planning

- Conducting counselling interviews

- Checking the quality and effectiveness of care

- Collaboration with relatives, doctors and specialist agencies
