For our Home Assistance department, we are looking for a dedicated individual from 1 December 2026 or by arrangement as

Employee Case Management Dementia 50-70%

Together with the Dementia Specialist Responsibility, you will establish our new case management. You will accompany clients and their relatives, provide professional support to our staff, and actively help shape the new service.

Your tasks

• Case management and coordination of care for clients with dementia
• Initial and needs assessments in the home environment as well as ongoing adjustment of care
• Professional support and advice to deployed staff in challenging care situations
• Consultation and involvement of relatives and key persons
• Collaboration with the Dementia Specialist Responsibility and external specialist agencies as well as participation in case discussions
• Co-design of processes, work tools, and standards for the new dementia service

Your profile

• Completed training in care or health, e.g. FaBe EFZ, FaGe EFZ or a comparable qualification
• Several years of experience in accompanying and caring for people with dementia
• Further training in dementia or willingness to undertake targeted further education
• Confidence and empathy in demanding dementia situations
• Ability to professionally assess care situations and competently advise staff
• Independent, structured, and communicative personality
• Driving licence category B required
