Your Tasks

You take on assignments in basic and treatment care with our customers
You independently conduct needs assessments with RAI-HC at the customer
You conscientiously carry out weekly controlling
You create care documentation and individual care planning and are responsible for regular, timely evaluation and accompany the progress of care goals
You lead your colleagues

What You Bring

Training as a Dipl. Nursing Specialist
MANDATORY: Successfully completed internal HC training
Several years of professional experience in the Spitex area or in long-term care
You are resilient, flexible, and used to working independently
You are process-oriented and think along
