Your tasks
• You take over case management for your own client base and coordinate the entire care process.
• You work closely in an interprofessional setting with our nursing expert, the needs assessment team, as well as doctors, therapists and other network partners.
• You advise and support clients and their relatives in complex care situations.
• You ensure professional nursing documentation and a high quality of care.
• You contribute your professional expertise and actively participate in the further development of our Spitex.
You bring with you
• Diploma as a nurse HF/FH or an equivalent qualification recognised in Switzerland
• Experience in outpatient care as well as in coordinating complex care situations
• Enjoyment in taking responsibility and actively shaping the care process
• Independent, structured and solution-oriented working style
• High social competence and enjoyment of interprofessional collaboration
• Driving licence category B
