About the job

• You are responsible for the medication supply and care of our patients upon hospital discharge
• You ensure holistic and competent advice for customers
• You actively support interdisciplinary collaboration

• You have a completed degree in pharmacy with a federal diploma or, as an EU citizen, an equivalent qualification
• You have preferably gained professional experience in a community pharmacy and have knowledge of ProPharma X
• You want to enthusiastically and focusedly optimise the medication process at the important interface (discharge) for the well-being of our patients
• You are interested in an interdisciplinary environment between hospital, public pharmacy, and hospital pharmacy and want to make an important contribution to increasing medication safety
