Your mission:

• Care and support: Responsibility for the entire care process of your clients
• Needs assessments: Conducting needs assessments with interRAI and creating care plans and case management
• Daily responsibility: Contact person for staff on tour
• Team reinforcement: Strengthening our palliative care team and actively participating in the further development and expansion of this specialised service

Your toolkit:

• Completed training as a Certified Nursing Specialist HF/FH with CAS/MAS in Palliative Care
• Professional experience in the outpatient setting is an advantage
• Enjoyment of working in an interdisciplinary team
• Motivation to help shape the palliative care of tomorrow
