Focus: Social Counseling and Case Management
With our diverse offerings in the area of career preparation (including "Top4Job") and work integration (day structure, IV measures), the "Supported Living" offer, partly financed by social services or disability insurance, as well as interdisciplinary counseling services (social counseling, psychotherapy, social coworking), we offer people in challenging situations a holistic and low-threshold support service that enables new perspectives for their lives.
Social Counseling
Short- or long-term, holistic and goal-oriented counseling for people aged 16-65 on the topics of self-organization & administration and budget & finances with the aim of improving their living conditions.
Case Management
Individual case management in the support process of participants in the various integration offers of Streetchurch with the goal of coordinating internal and external meaningful measures.
- Holistic and goal-oriented counseling for people (16 – 65 years) on the topics: self-organization & administration as well as budget & finances
- Individual case management of participants in the career preparation program "Top4Job" and "Low-threshold day structure"
- Cooperative collaboration with internal and external professionals
- Administration and documentation of the case management process
- Participation in team and offer-related meetings, case supervisions
- Ongoing studies in social work at a university of applied sciences, preferably second training internship
- Winning personality with confident appearance and joy in contact with a wide variety of people
- High social and self-competence
- Initiative, willingness to learn and ability to work in a team
- Efficient, goal- and result-oriented action with the necessary flexibility
- Microsoft Office skills
