Role description
- Conduct social situation assessments, follow-up, and social support for patients at our Toises psychotherapy center, taking into account the psychiatric dimension
- Work within a multidisciplinary team composed of psychiatrists, psychologists, nurses, neuropsychologists tasked with ensuring integrated psychiatric and psychotherapeutic follow-ups
- Coordinate the patient's social care in collaboration with the various center stakeholders and the network (CMS, CSR, SCTP, DGEJ, etc.)
- Conduct educational and support interventions in concrete steps (economic, administrative, professional, relational, legal, finding a place in an institution)
Your profile
- Bachelor HES in Social Work, social service orientation or equivalent degree
- Professional experience (minimum 2 years), mental health sector is an asset
- Knowledge of Swiss social services
- Competence in assessing the social situation of patients with mental disorders
