The KOGE specialist office (Health Coordination) supports the population of Glarus and service providers in the health and social sectors with information and advisory services as well as health-focused coordination / case management. It is a specialist office of the main health department.

To complement our team, we are looking for a

Care Coordinator / Case Manager 40–50 %

starting June 1, 2026, or by arrangement, with the workplace in Glarus.

Tasks

• Case management and coordination of complex care situations
• Advice and networking for affected persons, relatives, third parties, and service providers, especially regarding outpatient care and support
• Information and advice on health questions and services
• Participation in the further development of the specialist office
• Preparation of documentation, participation in statistics, and development of concepts

Requirements

• Care professional with HF or FH level training with additional MAS, CAS, NDS training in case management or similar
• Several years of professional experience in various care settings with a focus on Spitex
• Experience in interprofessional collaboration
• Digital competence (Office applications)
• Fluency in oral and written expression
• Committed, solution-oriented, and empathetic personality
• High presence and social competence
