About the job
The KOGE specialist office (Health Coordination) supports the population of Glarus and service providers in the health and social sectors with information and advisory services as well as health-focused coordination / case management. It is a specialist office of the main health department.
To complement our team, we are looking for a
Care Coordinator / Case Manager 40–50 %
starting June 1, 2026, or by arrangement, with the workplace in Glarus.
Tasks
- Case management and coordination of complex care situations
- Advice and networking for affected persons, relatives, third parties, and service providers, especially regarding outpatient care and support
- Information and advice on health questions and services
- Participation in the further development of the specialist office
- Preparation of documentation, participation in statistics, and development of concepts
Requirements
- Care professional with HF or FH level training with additional MAS, CAS, NDS training in case management or similar
- Several years of professional experience in various care settings with a focus on Spitex
- Experience in interprofessional collaboration
- Digital competence (Office applications)
- Fluency in oral and written expression
- Committed, solution-oriented, and empathetic personality
- High presence and social competence
