• Together with your team colleagues, you advise older people and their relatives, especially on independent living.
• You clarify the individual situations of older persons, taking into account the social environment, usually in the form of home visits.
• You mediate and coordinate support services.
• You make arrangements for the allocation of "vouchers for independent living".
• You actively participate in the further development of services and in projects in the field of aging.

• Completed studies in the social or health sector at HF or FH level.
• You actively contribute your very good knowledge and experience in the field of gerontology, outpatient nursing, and outreach systemic counseling activities.
• You meet all people openly, clearly, and empathetically.
• You advise with commitment, competence, and solution orientation.
