• Your field of activity
• You are responsible for optimal care and advice for our clients with a focus on dementia
• With case management, you take responsibility for all steps in the care process of your clients
• You take on coordination tasks with other involved services and actively participate in collaboration with our interprofessional team.

• Completed training as a certified nursing specialist (DNII / HF / FH)
• Experience in the care and support of people with dementia
• Empathetic, innovative, flexible and cheerful personality with strong social, communication and self-competence skills
• Flexibility and resilience
• Driving licence category B
