The project «AdvantAGE» is a pioneering, scientifically supported care model (Transitional Care) of the University Geriatric Medicine FELIX PLATTER in close cooperation with the Institute of Nursing Science at the University of Basel, as well as the Basel-Stadt Health Department. The aim of the project is to closely support very elderly patients during the transition from hospital back to the home environment, to avoid rehospitalisations and to promote independence.

Your tasks
Interface management: You independently manage the entire transition process from hospital discharge to consolidation at home.
Conducting home visits as an APN: You carry out structured home visits and perform comprehensive clinical assessments directly on site. You monitor the implementation and effectiveness of the measures initiated by the hospital and adjust them in collaboration with the general practitioners in case of changes in the health condition.
Interprofessional coordination & networking: You act as a central link between the hospital, general practitioner practices, the subsequent home care services (Spitex) and therapeutic services. You coordinate the continuation of treatment seamlessly and ensure the flow of information.
Patient education: You train affected persons and their relatives in self-management of chronic diseases and everyday barriers.
Research & development: You actively participate in the scientific process evaluation and quality development of the care model.

Your profile
Academic degree: Successfully completed studies as a nursing expert (Master of Science in Nursing / MScN)
Clinical expertise: Several years of solid professional experience in geriatrics, acute geriatrics, rehabilitation or outpatient long-term care (Spitex)
Personality: High personal responsibility, pronounced clinical decision-making competence and enjoyment of pioneering work in a research project
Communication: Strong interprofessional communication skills as well as an empathetic approach to older people
