Dipl. Nurse with Case Management - Dementia 60-100% (a)

Do you want to support people with dementia to live as safely and independently as possible in their familiar home for as long as possible? 

In the Memory Care Team at Spitex Zurich, you will provide holistic support to clients with cognitive impairments - from needs assessment through nursing care to advising relatives. You will take case responsibility and develop individual solutions that make everyday life easier for those affected and their environment. 

Your assignments take place during the day - usually between 07:00 and 16:00. 

• You take case management for people with dementia and steer the nursing process from needs assessment through planning to regular evaluation of the agreed measures.
• You care for and support our clients in their familiar home, promote their existing resources and help them maintain their independence for as long as possible.
• You advise and support relatives, coordinate collaboration with doctors and specialist agencies, and are an important contact person for all involved.
• You work closely within the Skill & Grade Mix framework with colleagues from various professional groups and ensure careful and complete nursing documentation.
• You network with internal and external specialists, contribute your knowledge to the team and support new employees, trainees and students during their induction. 

• Specialised working environment: You accompany people with dementia and work in an experienced Memory Care Team with high professional competence.
• Independent working: You take case responsibility and have the freedom to develop individual solutions for clients and relatives.
• Strong team support: Despite working independently, you are well connected professionally and benefit from regular exchange with colleagues and internal specialists.

• Completed nursing training at tertiary level
• Friendly and appreciative manner as well as physical and mental resilience
• Enjoyment of change and teamwork
• High sense of responsibility, independence and flexibility
• Good knowledge of the German language spoken and written, at least level B2
• Driving licence category B or willingness to be mobile with an electric bike
