Social Worker 50-80 %
for discharge and social counselling from 1 October 2026 or by arrangement.Your Challenge
- Advising patients, including their relatives, during the hospital stay to prepare for discharge and arranging follow-up solutions
- Interdisciplinary collaboration in the care of seriously ill and dying patients and their relatives
- Referral to external specialist services and medical aids
- Advising patients on insurance and legal matters, including clarifying financing issues, accessing financial support or untapped funding sources
- Obtaining cost approvals domestically and abroad
- Interdisciplinary cooperation with all nursing wards and medical as well as nursing services
- Collaboration and networking with aftercare institutions
- Participation in hospital projects
Your Profile
- Completed degree in Social Work FH or equivalent
- Experience in healthcare, social services, case management, or basic nursing training is an advantage
- Excellent communication skills
- Solution-oriented approach to crises and problem situations
- Strong social skills and empathy
- Willingness to work interdisciplinarily
- Eager to learn and interested in completing further training and education
- Professional handling of public services
- Motivated and committed personality, characterised by an independent working style and the ability to make decisions autonomously
