- Conducting interdisciplinary discussions with patients, relatives, and professionals
- Planning and organizing transfers to rehabilitation, cures, nursing homes, or care in the familiar environment
- Advising inpatient and outpatient patients as well as their relatives on psychosocial, financial, and social insurance issues
