What you achieve

• Responsibility for case management through comprehensive planning, control and evaluation of the nursing process to ensure continuous and optimal care.
• Close interdisciplinary collaboration with employees, specialist departments and doctors to ensure holistic care of clients and achieve the best possible treatment outcomes.
• Assuming resource- and goal-oriented nursing and care of clients by individually addressing their needs to ensure high nursing quality.
• Instruction, coaching and advice for nursing staff of other competency levels, promoting their professional development and supporting them in daily work.
• As an important link between management and the nursing team, contributing to ensuring holistic care and support of clients to achieve the best possible treatment outcomes.

What you bring

• Professional experience in the Spitex sector as a qualified nurse HF/FH, ideally in the role of case management and application of InterRAI HC
• Passion for the profession and motivation to make a positive impact on the market
• A strong team player who quickly gains access to clients and relatives through an open, communicative and empathetic manner
• An entrepreneurial mindset, high organisational skills and pronounced sense of responsibility
• Driving licence category B
