Your Tasks
- Seamless continued care and, if necessary, needs-based further referral of our multimorbid patients to appropriate institutions
- Close cooperation in the multiprofessional team
- Contact with relatives and caregivers and follow-up care providers, payers
Your Profile
- Completed studies in Social Work/Social Pedagogy (e.g., diploma, bachelor, master)
- Experience in clinical social services is desirable
