Your Tasks

At our clients, you will take on assignments in basic and treatment care
You will independently conduct needs assessments with RAI-HC at the client's location
You will diligently carry out weekly monitoring
You will create care documentation and individual care planning and be responsible for regular, timely evaluation and accompany the progress of care goals
You will guide your colleagues

What You Bring

Training as a Dipl. Nursing Specialist
MANDATORY: Successfully completed internal HC training
Several years of professional experience in the field of Spitex or in long-term care
You are resilient, flexible, and accustomed to working independently
You are process-oriented and think along
