About the job
In our geriatric psychiatry unit, currently with 16 beds, older people, usually from the age of 65, are comprehensively treated and supported. The focus is on the diagnosis, therapy and prevention of mental illnesses that occur in connection with age or may be favoured by age-related changes.
With professional competence, empathy and a holistic view, you accompany our patients through their individual care and treatment process. In doing so, you take responsibility for the professional management of care and work closely with the interprofessional treatment team as well as relatives and external partners.
Your area of responsibility
- Key worker role and nursing case management: You plan, manage and evaluate the care process of the patients assigned to you.
- Shift responsibility: You coordinate the shift professionally, set priorities and ensure a clear distribution of tasks and delegation within the nursing team.
- Psychiatric nursing: You assess the mental state, recognise changes early and act professionally and appropriately in crises and challenging situations.
- Geriatric psychiatric nursing: You support people with various clinical pictures such as dementia, delirium, depression, addiction disorders or psychoses as well as psychiatric and somatic multimorbidity.
- Work with relatives: You advise and support relatives and involve them appropriately in the treatment process.
- Interprofessional collaboration: You contribute your nursing expertise in ward rounds and case discussions and actively participate in setting and reviewing treatment goals.
- Discharge and transition management: Together with the treatment team, you develop sustainable follow-up solutions and work with Spitex, care homes, referrers and other external partners.
In doing so, you create a trusting and appreciative atmosphere in which our patients feel safe, taken seriously and professionally supported.
