Your mission:
- Care and support: Responsibility for the entire care process of your clients
- Needs assessments: Conducting needs assessments with interRAI and creating care plans and case management
- Daily responsibility: Contact person for staff on tour
- Team reinforcement: Strengthening our palliative team and actively participating in the further development and expansion of this specialized service
Your backpack:
- Completed training as a Certified Nursing Specialist HF/FH with CAS/MAS in Palliative Care
- Professional experience in outpatient settings is an advantage
- Enjoyment of working in an interdisciplinary team
- Motivation to help shape the palliative care of tomorrow
