Your Tasks
- You assist in preparing the hospital discharge or actively organize it.
- Organization and consultation of post-hospital care forms.
- Clarify and advise on psychosocial, legal, financial, and economic situations related to the hospital stay.
- You participate in defining common interdisciplinary care goals and implementing appropriate measures and resources.
- Clarification and advice on possible care and support services.
- In addition, you advise and support our experts in their daily professional work.
Your Profile
- You are a student of the Bachelor’s degree in Social Work (FH), have completed the central compulsory modules, and already have experience in the social work context. The first internship is already completed.
- Ideally, you bring medical knowledge or nursing experience.
- You are willing to expand your skills in one of our private hospitals and deepen your expertise further.
- You recognize difficult situations in time and can therefore prioritize correctly.
- You enjoy contact with people and treat them with respect, appreciation, and good manners.
