Your tasks
- Independently manage a versatile daily routine in outpatient care
- Ensure high quality of care
- Act as a link between employees, management, and other professionals
- Provide professional guidance and support to colleagues in daily work
- Interdisciplinary collaboration and coordination with external agencies
- Opportunity to take on case management and case responsibility
Your profile
- Certified nurse HF or FH
- Professional experience in outpatient care
- Motivation to initiate new things and maintain proven practices
- Open, communicative, and empathetic manner in dealing with colleagues, clients, and relatives
- Driver's license category B or enjoyment of cycling
